Vaccines are common procedures that are so successful in reducing illness that they’ve become
considered routine and harmless. While adverse reactions are uncommon and severe reactions are
considered rare, vaccinations are not without risk. Please read the information below and make an
informed decision before proceeding with the recommended vaccinations for your pet.Vaccines stimulate the immune system to create protection against specific diseases. Vaccines work to reduce the severity of future diseases and may prevent the disease infection completely. The most common adverse response to vaccinations is a mild, short term period of fever, sluggishness and reduced appetite. Pets may also experience pain and swelling at the injection site. These symptoms should last only a day or two but should be reported to us so we can keep track of the overall incidence of these reactions. Serious reactions
occur rarely and include vomiting, diarrhea, whole body itching or hives, swelling of the face or legs,
difficulty breathing or collapse. These symptoms need to be treated immediately with appropriate
medications. Rarely, your pet’s vaccination may result in injection site tumors. The cause of these difficult to treat tumors is not completely understood and cannot be predicted as to which pet will develop this most severe complication. Data shows these tumors will occur in 1 in 10,000 – 50,000 vaccinations. Treatment options are very invasive and not totally corrective. You will be responsible for any and all charges to treat any adverse reaction. We will tailor a vaccination plan for your pet based on his or her lifestyle and environmental risks. Duration of immunity for each vaccine in each individual animal is variable and cannot be predicted. We will abide by the vaccine manufacturers guidelines for vaccine intervals. We believe that the benefits of vaccinations outweigh the risks to the pets we see daily, and therefore recommend vaccinating your pet with the recommended vaccines.I have read and understand the above information about the benefits and risks associated with vaccinations. I give my informed consent to proceed with the recommended vaccinations and hold harmless the clinic, veterinarians and staff from any adverse reactions that may occur.
